Patient-Derived Organoids Predict Chemoradiation Responses of Locally Advanced Rectal Cancer

Abstract
This protocol describes the generation of patient-derived rectal cancer organoids (RCOs) from treatment-naive locally advanced rectal cancer (LARC) patients and their use to predict individual chemoradiation responses. RCOs recapitulate histological and genetic features of original tumors and can predict patient responses to neoadjuvant chemoradiation with 84.43% accuracy, making them a potential companion diagnostic tool for rectal cancer treatment.

- 1 Obtain rectal cancer biopsy tissue
- 2 Sort and preserve tissue fragments
- 3 Wash tissue in sterile PBS
- 1 Mince tissue into small fragments
- 2 Enzymatic digestion of tissue
- 3 Collect isolated tumor cells
- 4 Seed cells in Matrigel
- 5 Overlay with organoid culture medium
- 1 Observe and photograph organoids
- 2 Refresh culture medium every three days
- 3 Passage organoids when they reach 100 µm diameter
- 4 Clear Matrigel from organoid fragments
- 5 Re-seed organoid fragments in fresh Matrigel
- 1 Select healthy organoid cultures
- 2 Harvest and passage organoids into 48-well plates
- 3 Allow organoid maturation and stabilization
- 1 Expose organoids to 8 Gy irradiation
- 2 Replace culture medium post-irradiation
- 3 Measure organoid size every three days for 24 days
- 4 Assess organoid viability using CellTiter-Glo 3D assay
- 5 Calculate organoid size recovery ratio
- 1 Prepare 5-Fu treatment medium
- 2 Replace culture medium with 5-Fu treatment medium
- 3 Refresh drug-containing medium after three days
- 4 Photograph and measure organoid size every three days for 24 days
- 5 Assess cell viability using CellTiter-Glo 3D assay
- 6 Classify 5-Fu response
- 1 Prepare CPT-11 treatment medium
- 2 Replace culture medium with CPT-11 treatment medium
- 3 Refresh drug-containing medium after three days
- 4 Photograph and measure organoid size every three days for 24 days
- 5 Assess cell viability using CellTiter-Glo 3D assay
- 6 Classify CPT-11 response
- 1 Prepare organoids for cryopreservation
